Position Summary:
The Community Liaison is responsible for generating and responding to referrals from hospitals, physician practices, nursing facilities, and community partners for palliative care services. The Liaison will possess and demonstrate the ability to develop strong relationships with referral sources through outreach, education, and networking. Working with the physician and interdisciplinary team, the Liaison provides education to patients and caregivers about Hawaii Care Choices’ palliative care program, evaluates for eligibility, and assists with coordination of care and admissions support. Must effectively communicate Hawaii Care Choices’ mission, vision, and values to develop positive relationships with hospitals, healthcare providers, and community partners.
Duties & Responsibilities:
- Responsible for establishing strong & meaningful relationships with all stakeholders.
- Promote and market Hawaii Care Choices palliative care services through sales calls, in-service presentations, grand rounds, industry conferences/fairs, etc.
- Works with referral sources and office staff to ensure transparent execution of services.
- Establish top referral list and develop strategic call plan/routing system.
- Service existing accounts and prospect for new business.
- Develop strategies to approach potential customers and increase referrals.
- Create, organize, and execute educational presentations regarding palliative care services and eligibility.
- Strategize in the development of marketing & promotional efforts.
- Conduct patient admissions as needed.
- Assists admission team in gathering needed paperwork and/or patient information, follow up activities and/or resolving problems.
- Resolves customer complaints by investigating problems; deploying solutions; making recommendations to management.
- Meet established referral /admission target goals and sales quotas.
- Maintain detailed records of all contacts and meetings and create reports as needed -- including correspondence, activity reports, and monthly stats.
- Keep management informed by submitting activity and results reports, such as daily call reports, weekly work plans and monthly and annual territory analysis.
- Collects and evaluates statistical data.
- Assist in gathering information for budget reports and annual reports.
- Keep current with clinical data and presenting this data to health care professionals.
- Keep current with the activities of health care professionals and organizations in the marketplace.
- Monitor competition by gathering current marketplace information on products/services, offerings and activity.
- Make recommendations based on evaluating results and competitive developments.
- Maintain current knowledge of palliative care services, healthcare trends and community resources.
- Practice effective communication and interpersonal skills to enhance positive teamwork and accomplish goals/tasks.
- Serve as a member of agency committees as needed.
- Demonstrate initiative and work as a team player.
- Collaborate with the team and makes necessary recommendations to ensure customer needs are met and in line with the overall strategic initiatives of the organization.

Education & Experience:
- Bachelor’s Degree in Marketing, Communications, or Healthcare Administration is preferred.
- Minimum of one (1) year of experience in direct sales experience is preferred.
- Healthcare environment experience is preferred.
- Frequent use of private vehicles.
- Occasional use of computer, fax machine, hole puncher, paper shredder, paper cutter, copier and general office equipment.
Work Hours:
- Exempt position: requires flexibility and may be required to work after-hours, on weekends, and holidays as needed.
Pay $68,000 - $69,000/ annually
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
